Examination Details

Drivers have two options for scheduling medical exams. You can either visit a county clinic or make an appointment with your own physician. The Bay Area Vanpool Program provides a list of clinics that include many of the local facilities that offer this exam. There will be a fee for the DMV Medical Examination. Call several clinics if you wish to compare prices.

 

Be sure to have the examining physician fill out and sign both the US DOT Medical Examination Report form and the wallet-sized Medical Examiner's Certificate (provided by your doctor).

Proof of Medical Certification

You will be required to keep the wallet-sized Medical Examiner's Certificate with your license whenever you drive the van. Keep the 8 1/2" x 11" DMV Medical Examination Report form in your private records. Please do not send the DMV Medical Examination Report form to 511 Carpool. If you should lose the wallet-sized Medical Examiner's Certificate, your DMV Medical Examination Report form can be used as proof of certification.

Renewal

Please remember that the State of California requires that you renew your medical certification every two years.
